Exercise In Coronary Heart Disease Prevention

Prior to discharge, an individualised exercise and activity plan should be prescribed for phase II. In addition, resumption of sexual activity, driving and returning to work should be discussed. It is important to identify any misconceptions and to discuss patient goals to ensure they are safe and realistic. An exercise consultation, prior to discharge, should be carried out in order to help the patient plan and adhere to phase II activity and exercise (see Chapter 8). Advice should be individualised, clear and concise, as patients often have difficulty absorbing information in hospital. This may be linked to feelings of anxiety and depression, which can be a natural reaction following a cardiac event (SIGN, 2002).

Discharge And Home Healthcare Guidelines

Make sure that the patient understands the need to control risk factors through medication therapy, dietary modifications, exercise guidelines, stress-reduction methods, and follow-up care. Have the patient demonstrate an understanding of how to take medicine, how often, and the potential side effects. Emphasize the need for frequent monitoring of blood pressure and laboratory work. Explain which signs and symptoms indicate a need to contact the physician. These symptoms include headache, blurred vision, dizziness, sleepiness, confusion, and changes in sexual performance. If the patient experiences altered sexual performance after starting a medication, encourage her or him to notify the physician immediately to have the medication changed rather than just stopping it without consultation.

Avascular Necrosis Hyposthenuria

Must be distinguished from osteomyelitis (Table 7-9) Related to sickle cell vaso-occlusion of mesenteric blood supply and infarction in the liver, spleen, or lymph nodes that results in capsular stretching. Table 7-10 lists the differentiation between painful abdominal crisis and acute abdomen. May include convulsions, meningeal signs, blindness, radiculopathy, vertigo and acute mental syndrome, cerebral infarction. Incidence 7-29 mean age of onset 7.7 years and increased incidence of subarachnoid hemorrhage. Figure 7-6 depicts the investigations and treatment of neurologic deficit Presents with chest pain that may be pleuritic, unexplained dyspnea, and fever differential diagnosis from pneumonia may be difficult (Table 7-11) it may be associated with full-blown sickle cell girdle syndrome Predisposing factors sexual intercourse, masturbation, infection, and local trauma impotence in many cases Painless and usually mild defect is papillary necrosis Sudden, painful enlargement of liver...

Obsessivecompulsive Disorder

However, there are more cogent reports on other negative consequences from taking SSRIs. One of the most prevalent problems arising from SSRI use is the high incidence of sexual dysfunction, most likely due to serotonin's role in regulation of sexual hormones. Associated dysfunctions include the inability to maintain an erection, inability to reach orgasm, and loss of libido. When Eli Lilly first published a list of side effects from Prozac's clinical trials, the reported rate of sexual problems was 3-5 percent it is now known to have an incidence as high as 50 percent and because of this many people refuse to even try SSRIs. Some psychiatrists combat this problem by also prescribing drugs which alleviate sexual dysfunction (such as Viagra or dopamine-enhancing drugs). A few patients even take drug holidays, because the side effects of most SSRIs go away immediately after the medication is stopped while the therapeutic effects last for days. Patients are able to do this because the...

Safety Issues with Cannabis Based Medicines

Might have been a trigger for this event (Mittleman et al. 2001). Unfortunately, only 124 of the 3,882 patients surveyed admitted to smoking marijuana. The risk of infarction appeared to be elevated 4.8 times over baseline during the 60 min following marijuana use but decreased rapidly thereafter. However, this conclusion has been much criticised, not least because the sample of subjects upon which it is based (those who had smoked cannabis within 1 h of infarction) amounted to only 9 individuals, of whom 3 admitted at least one other triggering activity (e.g. cocaine use or sexual intercourse). Epidemiological data on 65,000 patients in the San Francisco Bay Area do not support an increased risk of cardiac events in cannabis smokers (Sidney et al. 1997).

Within Group Comparison of SSRIs

Direct comparisons between the SSRIs specifically looking at sexual dysfunction are few. Modell et al51 gave an anonymous questionnaire to 107 outpatients on antidepressants. 73 of patients on fluoxetine, paroxetine or sertraline reported a decrease in libido, arousal, duration or intensity of orgasm below their premorbid levels, but there was no difference between these three SSRIs. In a head-to-head study with sertraline and fluvoxamine, which specifically looked at sexual function, 28 of sertraline (mean dose 137 mg day) patients developed dysfunction compared to only 10 of patients on fluvoxamine (mean dose 124 mg day).41 Prerelease data reveals slightly different rates for the SSRIs but it is likely that different criteria were used and thus the rates cannot be directly compared. Often the focus was on abnormal ejaculation which may explain why some rates have ranged from 1-2 in women to 21 in men.

Therapeutic Interventions For Adults With Pd And Impaired Sexual Function

Impaired sexual function in PD patients is most likely multifactorial. Partners and couples should be individually assessed to guide therapeutic intervention. Depression, physical disability, and autonomic dysfunction may contribute to the increased incidence of ED in men with PD.17 In men with PD, there is limited discussion of first line pharmacological therapies for ED. Treatments with testosterone gel47 and sildenafil48,49 have been described. Testosterone is thought to stimulate libido in the central nervous system.50 Erections in response to erotic visual stimuli may be partially androgen dependent.51 Animal and human studies have found that low normal range concentrations of testosterone are sufficient to maintain sexual activity.52 Testosterone deficiency is found in 20 to 25 of men over age 60.47 Testosterone deficiency can result in depression, fatigue, decreased libido, and decreased work performance. Okun et al.47 retrospectively analyzed the effect of testosterone...

Vertebrobasilar Stroke Syndromes

Transient global amnesia (TGA) is an episode of total amnesia that lasts little more than 24 hours. Patients classically ask many questions repeatedly. During the episode, there are no visual, motor, sensory, or brain stem abnormalities. After the episode, the individual has no recall for the events during the period of TGA, but there are no other cognitive residuals. The mechanism of TGA is not known. Theories advanced suggest either medial temporal lobe seizure activity or a vascular phenomenon, such as posterior cerebral artery territory ischemia involving the medial temporal lobes bilaterally. There seems to be an association with strenuous exertion, sexual intercourse, and emotional stress. y

Indications for family therapy

To reiterate, family therapy may not be the only treatment of choice. Thus, in helping a disturbed family struggling to deal with a schizophrenic member, supportive therapy and medication for the patient are likely to be as important as any family treatment. Similarly, an indication for family therapy does not negate the possibility of another psychological approach being used for one or more family members. For instance, an 18-year-old adolescent striving to separate may benefit from individual therapy following family treatment (or in parallel with it), while the parents may require a separate programme to focus on their sexual relationship.

Abnormal Uterine Bleeding

History of the Present Illness Last menstrual period, age of menarche regularity, duration and frequency of menses amount of bleeding, number of pads per day passage of clots postcoital bleeding, intermenstrual bleeding abdominal pain, fever, lightheadedness, sexually active, possibility of pregnancy, birth control method, hormonal contraception.

Sexual Impotence

Sexual impotence refers to a man's penis not erecting or erecting without enough hardness. It is usually caused by disturbance of the controlling function of the penis erection in the cerebral cortex or spinal central nervous system. Neurasthenia, chronic backaches and excessive sex life can also cause the disease.

Sexual Functioning

Impaired sexual functioning is a well-recognized complication of type 2 diabetes in men and women. The prevalence of erectile dysfunction (ED) in the overall population between the ages of 40 and 70 years is 52 , while the prevalence in men with diabetes is as high as 75 (92-95). Moreover, diabetic men develop ED at an earlier age than men without diabetes (2). In women, type 2 diabetes has been shown to impact sexual desire, orgasmic capacity, lubrication, sexual satisfaction, sexual activity, and relationship with sexual partner (96). Relationship problems may be a primary or aggravating factor in sexual functioning (97). Performance anxieties and relationship problems have been identified as potential problem areas that may need sensitive investigation (98). Sildenafil citrate and related medications are well-tolerated and effective in improving ED in men with type 2 diabetes, even in patients with poor glycemic control and chronic complications (99). The rates of adverse events,...

Sildenafil

GENERAL Preparations Brand name Viagra only. Used in Impotence from any etiology, including that caused by psychiatric medications. Its use in women with difficulty achieving orgasm is under investigation. Mechanism of action Inhibits phosphodiesterase type 5, causing increased levels of guanosine monophosphate, relaxation of smooth muscle, and greater blood flow to the penis, enhancing erection during sexual stimulation. PRECAUTIONS Warnings Deaths have been associated with the simultaneous use of Viagra and nitrates (used both in prescription medicines for heart disease, such as nitroglycerin, Nitrobid, or Isordil, and drugs of abuse such as amyl nitrite). Decreased or a loss of vision can occur with sildenafil due to nonarteritic anterior ischemic optic neuropathy (NAION). Don't use if you have Heart disease, as there is a degree of heart risk associated with sexual activity. Use with extra caution if you have Retinitis pigmentosa, as there is the potential for adverse effects with...

Vardenafil

PRECAUTIONS Warnings Deaths have been associated with the simultaneous use of this type of medication and nitrates (used in prescription medicines for heart disease such as nitroglycerin, Nitrobid, or Isordil, and recreational drugs such as amyl nitrate). Decreased or a loss of vision can occur with vardenafil due to nonarteritic anterior ischemic optic neuropathy (NAION). Don't use if you have Heart disease, as there is a degree of heart risk associated with sexual activity. Use with extra caution if you have Kidney or liver problems, as vardenafil is not cleared from the body as efficiently retinitis pigmentosa, as there is the potential for adverse effects with the use of vardenafil. Tests needed before starting None. Alcohol The combination can cause low blood pressure, elevated heart rate, headaches, and dizziness. DOSING Adults The usual starting dose is 10 mg at least 60 minutes before sexual activity, no more than once a day. Subsequent doses up to a maximum of 20 mg day can...

Caffeine

To be a safe agent in most groups of patients (29). Sildenafil is an oral medication, licensed for clinical use in three doses 25, 50, and 100 mg. It should be taken 1 h before sexual activity, after which time plasma levels should be at a peak. Further time may be required if taken with a high-fat meal as this increases transit time in the gut, but no dose adjustment is necessary (24). After starting on 50 mg per day, the dose can be titrated up or down, depending on tolerance and efficacy, and should not normally exceed 100 mg, as adverse events are more likely to occur at higher doses with no proven increase in efficacy (30). Cardiovascular disease In patients with cardiovascular disease, it is important to determine whether any treatment for ED would be contraindicated (e.g., where sexual activity is inadvisable). Men with cardiovascular disease tend to have an increased number of risk factors such as smoking and diabetes. These men are also quite prone to depression, which...

Alkyl Nitrites

The alkyl nitrites are volatile yellowish clear liquids that have a distinctive sweet smell. All the nitrites have vasodilatory properties and are used as a euphoric relaxant within the dance culture and to relax the anal sphincter and enhance sexual performance. The effect of inhaling the vapor, usually from the bottle or poured onto a cloth, is instantaneous and short-lived, resulting in a rush, but adverse effects, such as dizziness, flushing, tachycardia and palpitations, headache, cold sweats, and hypotension, may occur (115,116). Swallowing of volatile nitrites as opposed to inhaling them may result in severe methemoglobinemia (117).

On presentation for yearly exam, a healthy, non-sexually active post-menopausal 60-year-old female gives a history of having had normal yearly mammograms and normal yearly Pap smears over the past 10 years, but never an endometrial tissue sample or any screening test for ovarian cancer. The most clearly indicated cancer screening evaluation on today's visit is

Cyproheptadine

GENERAL Preparations Generic and brand name Periactin in pills and liquid form. Class Antihistamine. Used in Cyproheptadine improves the ability to achieve orgasm in some people who experience sexual dysfunction induced by SSRIs. It can increase the rate of weight gain in hospitalized patients with anorexia. Mechanism of action Blocks postsynaptic histamine and serotonin receptors. DOSING Adults For sexual dysfunction, the usual dose is 4-12 mg one to two hours prior to initiating sexual activity. To increase weight gain in anorexia, amounts up to 32 mg day in divided doses are used.

Pedophilia

A 32-year-old female is brought to the emergency department following a fainting episode she claims that she developed dizziness and shortness of breath during sexual intercourse. HPI Upon further questioning, she admits that her male partner, with her consent, routinely fastens a belt around her neck during intercourse, adding that she also submits routinely to being bound, choked, and whipped by her partner during sexual activity. She had fantasies of being beaten and choked during sex during adolescence but began to act on those fantasies only 2 years ago.

Tadalafil

PRECAUTIONS Warnings Deaths have been associated with the simultaneous use of this type of medication and nitrates (used both in prescription medicines for heart disease such as nitroglycerin, Nitrobid, or Isordil, and drugs of abuse such as amyl nitrate.) Decreased or a loss of vision can occur with tadalafil due to nonarteritic anterior ischemic optic neuropathy (NAION). Don't use if you have Heart disease, as there is a degree of heart risk associated with sexual activity. Use with extra caution if you have Kidney or liver problems, as tadalafil is not cleared from the body as efficiently retinitis pigmentosa, as there is the potential for adverse effects with the use of tadalafil.
